Who Benefits from BHRT?
Perimenopause (typically 35–50): hormonal fluctuations causing irregular periods, sleep disruption, mood shifts, brain fog, weight gain, decreased libido, hot flashes. BHRT can stabilize symptoms before menopause.

The Bespoke BHRT Protocol
Baseline labs: estradiol, progesterone, total + free testosterone, DHEA, FSH/LH, comprehensive thyroid (TSH, free T3, free T4, reverse T3, antibodies), cortisol (4-point salivary or AM serum), fasting insulin, vitamin D, lipid panel.
Protocol design: bioidentical estradiol (transdermal cream, patch, or pellet), micronized progesterone (oral or vaginal), testosterone (low-dose female protocol, typically subcutaneous), DHEA, thyroid replacement if indicated. Each medication is dosed to your specific lab values and symptoms.
503A pharmacy compounding: bioidentical hormones are compounded by U.S. state-licensed 503A pharmacies to the dose Dr. Indianos specifies. Compounded preparations are not FDA approved.
Monitoring: follow-up labs at 6 weeks, then every 3–6 months. Symptom tracking. Annual comprehensive review. Adjust hormones, dose, route based on response.
What We Can and Cannot Tell You About BHRT and Risk
The 2002 Women’s Health Initiative results reshaped how hormone therapy is discussed, and much of what people took from it was applied more broadly than the study supported. That is fair to say. What is not fair to say is that compounded bioidentical hormones have been shown to be safer.
Here is the honest position. Compounded preparations are not FDA approved, and they have not been through the large outcome trials that FDA-approved hormone products have. That means nobody, including us, can tell you what a compounded protocol does to your long-term cardiovascular, bone, breast, or cognitive risk, because the studies that would answer that have not been done on these preparations.
Dr. Indianos goes through your individual risk factors with you before anything is prescribed, including family history of breast cancer, clotting history, smoking, and cardiovascular disease. Where an FDA-approved product is the better choice for you, she will say so and prescribe it. Compounding is a tool for people who need a dose or a route that commercial products do not offer, not a default and not an upgrade.
