Vitamin B12 (cobalamin) is essential for red blood cell formation, neurological function, DNA synthesis, and energy metabolism. The body cannot synthesize B12; it must come from diet (animal products) or supplementation. Many adults — especially over 50, vegetarians/vegans, those on metformin, PPIs, or with autoimmune gastritis — have suboptimal B12 levels even with apparently normal lab ranges.
Oral B12 absorption is limited by gastric intrinsic factor and gut health. Sublingual and oral supplements help some patients, but many need injection-based supplementation to achieve therapeutic levels. B12 injections deliver bioavailable cobalamin directly to tissues.
Methylcobalamin: the active, methylated form. Supports the methylation cycle directly — particularly relevant for patients with MTHFR gene variants. Best for cognitive, neurological, and mood support.
Hydroxocobalamin: longer-acting form, often retained in tissues longer than cyanocobalamin. Used for chronic fatigue and pernicious anemia.
Cyanocobalamin: the most common pharmacy form. Inexpensive, well-absorbed, suitable for most patients without methylation concerns.
Lipo-B / Lipotropic B12: B12 combined with methionine, inositol, and choline (“MIC”) for fat metabolism support. Often used as adjunct to weight loss programs.

Loading phase: weekly injections for 4–8 weeks for active repletion or symptomatic deficiency.
Maintenance: every 2–4 weeks based on labs and symptoms. Some patients prefer monthly; some weekly indefinitely.
Dosing: typical 1000 mcg IM or subQ. Higher doses (2500–5000 mcg) for severe deficiency or absorption issues.
Lab follow-up: recheck B12 (and methylmalonic acid for functional B12 status) at 8–12 weeks to confirm response.
