Testosterone replacement therapy isn’t just “low T → inject testosterone.” Done right, TRT is comprehensive endocrine optimization: free testosterone, total testosterone, estradiol, SHBG, LH, FSH, DHEA, thyroid markers, and metabolic health are all part of the picture. Protocol selection: testosterone cypionate or enanthate IM injection (twice-weekly typical, sometimes daily subcutaneous for steadier levels), HCG to preserve fertility / testicular function, anastrozole only if estradiol management requires it (most men don’t need it).
Monitoring: follow-up labs at 6 weeks, 12 weeks, then quarterly. Adjust dose based on lab response, symptoms, hematocrit. Annual comprehensive review.
