What Thorough TRT Care Involves
Good TRT is more than "low T, so inject testosterone." It starts with a full endocrine picture: free and total testosterone, estradiol, SHBG, LH, FSH, DHEA, thyroid markers, and metabolic health. Done carelessly, TRT can raise hematocrit, suppress fertility, or convert too much testosterone to estradiol, which is why Dr. Indianos looks at the whole picture instead of a single number.
Who Needs TRT?
Symptomatic low testosterone:
fatigue, low libido, erectile dysfunction, low mood, poor sleep, low motivation. These symptoms together with a total testosterone confirmed low on two early-morning draws, interpreted against standardized reference ranges (Endocrine Society lower limit 264 ng/dL; AUA 300 ng/dL).
Age-related decline:
testosterone does drift down with age in most men. That drift on its own is not a diagnosis and it is not automatically something to treat. What matters is whether your levels are actually low, whether you have symptoms that track with them, and whether something else explains those symptoms better. FDA-approved testosterone products are approved for specific medical causes of low testosterone, not for aging.
Post-finasteride syndrome:
some men experience persistent low T after finasteride/dutasteride. Comprehensive lab workup determines if TRT is appropriate.
What if my labs come back normal?
Then you have a real answer, and a physician who keeps working it with you.
Testosterone is a Schedule III controlled substance. It is prescribed to correct a documented deficiency, not to push a normal level higher. Major endocrine guidelines advise against treating men with normal testosterone, and treatment carries real tradeoffs for fertility and hematocrit.
If your labs are normal, Dr. Indianos will tell you so and look at sleep, weight, thyroid, iron, and alcohol instead, because those are more often the actual answer.

Baseline labs:
total testosterone, free testosterone, SHBG, estradiol (sensitive assay), LH, FSH, DHEA, prolactin, PSA, hematocrit, lipid panel, fasting insulin, HbA1c, comprehensive thyroid (TSH, free T3, free T4, reverse T3), vitamin D.

Protocol selection:
testosterone cypionate or enanthate IM injection (twice-weekly typical, sometimes daily subcutaneous for steadier levels), hCG may be used alongside TRT, off-label, to help maintain fertility-related hormone signaling; discuss fertility plans before starting. Anastrozole only if estradiol management requires it (most men don't need it).

Monitoring:
follow-up labs at 6 weeks, 12 weeks, then quarterly. Adjust dose based on lab response, symptoms, hematocrit. Annual comprehensive review.

Monitoring and Follow-Up
Response to treatment varies from person to person, and some men notice little or no change. Dr. Indianos rechecks labs at 6 weeks, at 12 weeks and quarterly after that, and adjusts or stops treatment based on your lab response, your symptoms and your hematocrit. Treatment is continued only while it remains clinically appropriate.
