3. Medical Consent and Disclaimer
All patients are required to complete medical intake forms and provide accurate health information before receiving treatment. 4. Appointments and Scheduling

We require at least 24 hours’ notice for cancellations or rescheduling. Late cancellations or missed appointments may be subject to a cancellation fee. Repeated no-shows may result in the requirement of a deposit for future bookings.
