NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every cell in your body. According to the NIH Office of Dietary Supplements, more than 400 enzymes require it, which is more than for any other vitamin-derived coenzyme, and it is central to the reactions that transfer energy from carbohydrates, fats, and proteins to ATP. Research has observed that cellular NAD levels tend to be lower with age. What follows from that observation for an individual patient is a far less settled question, and we are not going to describe it as settled. NAD+ is a coenzyme found in all living cells. It plays essential roles in cellular energy production (ATP via mitochondrial respiration), DNA repair (PARP and sirtuin activation), cellular signaling, and metabolic regulation.
Cellular NAD levels vary between people, and research has observed that they tend to decline with age. You will see confident percentages attached to that claim across this industry, usually a figure like half by age 50. We are not going to repeat those numbers, because the study most often cited for them measured a single tissue type in 49 people and never stated a percentage at any age. The honest version is the direction, not a number. Raising a measured NAD level is also not the same thing as producing a change a person feels, and that distinction is where most marketing on this subject quietly stops.
Two different things get blended together in most writing on this subject, so we will separate them.
What is established biochemistry. NAD is required by the enzymes that transfer energy from food to ATP. It is a substrate for sirtuins and PARP enzymes involved in DNA repair and gene expression. These are facts about what the molecule does inside a cell, and they are not controversial.
What is not established. That giving a specific person more of it produces a specific felt result. In a 2025 randomized controlled trial, participants taking a NAD precursor raised their blood NAD levels roughly threefold, and still showed no statistically significant benefit over placebo on cognition, fatigue severity, sleep quality, or mood. Separately, a pooled analysis of eight randomized trials found no significant effect on blood sugar or lipid measures. There is currently no randomized controlled trial showing that intravenous NAD improves energy, focus, or recovery in humans.
So here is our position. NAD+ IV therapy is not a treatment for fatigue, for aging, or for any medical condition, and we will not describe it as one. Patients ask us about it for general wellness support, and Dr. Indianos will tell you at consultation whether she thinks it is a reasonable use of your money for what you are actually trying to address. Sometimes the answer is no. Individual results vary and no outcome is guaranteed.
Pre-treatment physician assessment. Personalized dosing based on your goals, your tolerance, and baseline labs. Sessions typically run 2 to 4 hours, administered slowly and supervised throughout. Where Dr. Indianos judges an initial loading schedule appropriate, sessions may run daily for a short period, followed by monthly or quarterly maintenance. Pre-infusion vitals and tolerance check. Post-infusion observation. Dose, pace, and schedule are physician decisions made after screening, not options selected from a menu. Some patients pair NAD+ IV sessions with cold plunge, infrared sauna, or red light therapy.

Professionals managing demanding schedules. Whether NAD+ IV therapy suits you is a clinical decision made at consultation, not something to determine in advance from a web page.
Contraindications: NAD+ IV therapy is not appropriate for everyone. Dr. Indianos screens for contraindications at consultation, including active malignancy, significant kidney or liver disease, cardiovascular history, and current medications. Safety data in pregnancy and breastfeeding is insufficient, which is grounds for caution rather than evidence of harm. Screening is the point of the consultation, not a formality attached to it.
NAD is a large, charged molecule and is not well absorbed intact from the digestive tract. That is why oral products in this category almost always supply precursors, such as nicotinamide riboside or nicotinamide mononucleotide, and rely on your body to build NAD from them rather than delivering NAD itself. An infusion places the molecule past digestion entirely.
That is an argument about delivery, not an argument about outcome. Bypassing the gut means more of the molecule reaches circulation. It does not establish that the result is better, and it does not make an unproven outcome proven. We state the distinction plainly because a great deal of marketing in this category depends on blurring it.
NAD+ IV is generally well tolerated, but the infusion itself is not always comfortable, and we would rather you hear that from us first. In the small published study that has looked at this directly, patients receiving intravenous NAD reported gastrointestinal symptoms, increased heart rate, and chest pressure during infusion, severe enough that infusions were slowed considerably. Symptoms resolved once the infusion finished. This is exactly why we titrate slowly, why sessions run longer than patients expect, and why you are supervised throughout. Tell your clinician immediately if you feel unwell at any point. This is not a complete list of possible effects, and NAD+ IV therapy is not appropriate for everyone.
NAD for intravenous use in the United States is a compounded preparation. Compounded medications are not FDA approved, because compounding is a pharmacy practice rather than an FDA drug-approval pathway, and the agency does not verify the safety, effectiveness, or quality of compounded preparations before they are marketed. That is a plain statement of regulatory status, not an accusation against any pharmacy or clinic.

NAD+ IV therapy has not been studied in pregnancy or breastfeeding. There is no human safety data either way, which is why we exclude it in those circumstances. That is a gap in the evidence, not a finding of harm.
