Low Testosterone Symptoms in Men
Fatigue, low libido, brain fog and weight gain can point to low testosterone, or to something else entirely. A lab check with Dr. Carolyn Indianos, MD tells you which.

Signs worth noticing
Low testosterone doesn't usually show up as one dramatic sign. It shows up as a handful of things that each seem small on their own, and easy to blame on age, stress, or a bad month, until you notice they've all been true for a while. Here's what to look for.

Symptoms men bring to us
Fatigue that sleep doesn't fix
The most common complaint we hear. Not the tired you feel after a late night, but a low-grade tiredness that's there even after eight hours, that coffee barely touches, and that's been building for months.
Is this you? If you're sleeping enough by the clock but still dragging through the afternoon most days, testosterone is worth ruling in or out.
Low libido and reduced sexual performance
A drop in sex drive, difficulty with performance, or both. One of the more direct signals of low testosterone, though not the only possible cause, and rarely the only symptom present when testosterone is actually the issue.
Is this you? If this change has been gradual and sustained rather than tied to a specific stressor, it's worth a lab check.
Muscle loss and reduced strength
Low testosterone can contribute to reduced muscle mass. An evaluation can help identify the cause and whether treatment is appropriate.
Is this you? If you've noticed muscle loss without an obvious reason, it's worth a lab check.
Mood changes, irritability, and low motivation
Associated with flatter mood, shorter patience, and a general loss of drive, the kind of change people around you might notice first.
Is this you? If you feel like a duller version of yourself and it's not explained by an obvious life event, it belongs on the list.
Poor sleep and disrupted sleep quality
The relationship runs both directions: low testosterone can worsen sleep, and poor sleep can further suppress testosterone.
Is this you? If your sleep has quietly gotten worse over the past year without a clear cause, this is one of the less obvious threads to pull.
Brain fog and difficulty concentrating
Trouble focusing, slower recall, a general mental fuzziness that wasn't there before. Dismissed most often, but a documented pattern in men with low testosterone.
Is this you? If your focus has slipped and you can't point to why, it's worth adding to the conversation.
Unwanted weight gain, especially around the midsection
Low testosterone shifts body composition toward more fat and less muscle, first around the abdomen. Often the symptom that finally prompts men to ask about testing.
Is this you? If your weight has crept up despite no real change in habits, concentrated in one area, that's common with low T.
Poor recovery from training or daily life
Longer soreness, injuries that take longer to heal, a sense your body doesn't bounce back like it used to.
Is this you? If two days off used to be enough and now it takes four, that's a meaningful change, not just getting older.
Why these symptoms don't tell the whole story
Every symptom above has other possible causes.
Fatigue can come from sleep apnea, thyroid issues, or depression. Weight gain from a dozen unrelated factors. Brain fog from stress or poor sleep. That's the honest limitation of a symptom list: it tells you what to watch for, not what you have.
The only way to know if low testosterone is behind what you're feeling is a blood test, reviewed by a physician who also examines you and takes your full history into account.
Self-diagnosing from a checklist and jumping to treatment is exactly the shortcut a real evaluation is designed to prevent.
Our approach
What causes low testosterone
Testosterone naturally declines with age, but a meaningful drop below normal range can also stem from obesity, poor sleep, chronic stress, certain medications, thyroid disorders, and some underlying medical conditions. This is why testing and a physician read of your full picture matter more than the symptom list alone.

Frequently Asked Questions
Common questions about treatment, protocols, and Bespoke’s clinical approach.
Common symptoms include persistent fatigue, low libido, reduced muscle mass, mood changes or irritability, brain fog, disrupted sleep, unwanted weight gain, and slower recovery. Most men experience a combination of several, not just one.
It can be. Fatigue that persists despite adequate sleep is one of the most commonly reported symptoms, but it has many other possible causes, so it’s not diagnostic on its own.
The only reliable way is a blood test measuring total and free testosterone, reviewed alongside your symptoms and health history by a physician.
Natural aging is the most common factor, but obesity, chronic stress, poor sleep, certain medications, and some medical conditions can also lower levels below normal range.
