Microneedling with PRP — The “Vampire Facial,” Done Medically

Microneedling alone creates controlled micro-injuries that trigger your skin’s wound-healing cascade — new collagen, elastin, and epidermal cell turnover. Adding PRP (platelet-rich plasma) supercharges that response: the growth factors in your own blood platelets accelerate healing, deepen the collagen response, and enhance results.

Popularly called the “Vampire Facial,” microneedling with PRP at Bespoke Aesthetics is a medical procedure performed by Dr. Carolyn Indianos, MD — not a spa treatment. Your blood is drawn, processed in a sterile centrifuge to isolate platelets, then applied during and after microneedling so growth factors penetrate through the channels created by the device.


What Microneedling + PRP Treats

Acne scarring. This is where microneedling has its best evidence, particularly for rolling and boxcar scars. Ice-pick scars respond least well and often need a different approach entirely, which Dr. Indianos will tell you at consultation rather than after you have paid for a series. Published studies report improvement across multiple sessions using assessment scales, and those numbers do not translate into a percentage you can expect on your own face. What we can commit to is photographing your baseline so you can judge the result yourself.

Uneven pigment. Post-inflammatory pigmentation, the kind left behind after acne or irritation, often improves as the skin remodels. Melasma is a different problem and needs caution: it can be made worse by heat and by aggressive treatment, and it is managed rather than cured. If melasma is your main concern, Dr. Indianos will tell you whether this is the right tool or the wrong one before you book a series.

Fine lines and crepey skin — collagen induction tightens the dermis and reduces fine lines, especially around the eyes, mouth, and neck.

Texture and pore size — refined skin texture and visibly smaller pores.

Stretch marks — effective on early stretch marks (still red/purple) and even mature ones with multiple sessions.

Hair thinning. Scalp microneedling with PRP is used for early pattern hair thinning. The evidence base is developing rather than settled, study quality varies, and this is not FDA approved for hair. It works best early, which is the honest and inconvenient part: by the time a follicle is gone, nothing on this page brings it back. Dr. Indianos will tell you at consultation whether you are early enough for this to be worth your money.


The Bespoke Protocol

Step 1: Consultation. Dr. Indianos reviews your skin, history, medications, and goals. Microneedling has specific contraindications (active infection, active acne in the treatment area, certain medications) that must be screened.

Step 2: PRP draw. A standard blood draw (1–2 vials) is processed in a sterile centrifuge to separate platelet-rich plasma from red and white blood cells.

Step 3: Topical numbing. 30 minutes of topical anesthetic for comfort.

Step 4: Microneedling. Medical-grade device (not the “derma roller” sold at drugstores) creates precise, controlled micro-channels at depths matched to your skin and treatment goal (typically 0.5–2.5mm depending on area).

Step 5: PRP application. Your processed PRP is applied during and after microneedling. The microchannels allow growth factors to penetrate where they would otherwise sit on the skin surface.

Step 6: Recovery. Skin is pink to red for 24–48 hours, like a mild sunburn. Mild flaking days 3–5. Full recovery in 5–7 days.


How Many Sessions and How Often

Most indications require 3–6 sessions spaced 4–6 weeks apart for optimal results. Acne scarring may need 6–8 sessions. Maintenance is typically 1–2 sessions per year after the initial series.

Results build across a series rather than arriving after one appointment. Some people notice smoother texture early and some do not, which is why this is sold as a series and photographed at baseline. The collagen remodeling that does the real work happens over months, not days, and continues for up to a year after your last session.
