What Are Exosomes?

Exosomes are 30–150 nanometer extracellular vesicles produced by mesenchymal stem cells (MSCs). They function as the cell’s signaling cargo, carrying growth factors (TGF-β, VEGF, FGF), cytokines, lipids, mRNA, and microRNA that modulate inflammation, stimulate fibroblast activity, and orchestrate tissue regeneration. Unlike whole stem cells, exosomes carry the regenerative signal without the cellular material, which means a more consistent product and easier sterile handling.

How Bespoke Sources Exosomes

Sourcing matters more in regenerative medicine than almost any other treatment category. Bespoke uses exosome products from FDA-registered, AATB-accredited tissue banks with full chain-of-custody documentation. The products we use are isolated from screened donor MSCs, processed under cGMP standards, and shipped under cold chain. We do not use research-grade, undisclosed-source, or imported products.

What We Treat with Exosomes

Bespoke uses exosome therapy for three primary clinical indications:

1. Facial Skin Rejuvenation

Combined with microneedling or RF microneedling, exosomes drive a faster, smoother resurfacing recovery and amplified collagen response. Targets fine lines, post-acne texture, hyperpigmentation, and overall skin quality. Most patients book a series of 3 sessions spaced 4–6 weeks apart.

2. Scalp & Hair Restoration

Microchanneling or scalp microneedling with exosome application is one of the scalp approaches patients ask about. Exosome products are not approved by the FDA for hair restoration or for any therapeutic use, and the FDA has issued public safety warnings about exosome products marketed for aesthetic and regenerative purposes. Whether any scalp approach is appropriate, and what it would involve, is determined with Dr. Indianos at consultation.

3. Post-Procedure Recovery

Topical exosomes work well after microneedling, RF, biostimulator injections (Sculptra, Radiesse), or laser. They are used in post-procedure protocols to support the skin's normal recovery process; individual results vary.

Exosomes vs. PRP, What’s the Difference?

PRP (platelet-rich plasma) and exosomes both deliver growth factors, but they come from different biological sources and behave differently:

  • PRP is autologous, concentrated from your own blood. Growth-factor content depends on your physiology, age, nutrition, and that day’s draw.
  • Exosomes come from young, healthy donor MSCs, standardized concentration, no donor-day variability.
  • For patients over 50 or those with depleted regenerative reserves, exosomes often outperform PRP on outcome consistency.
  • For younger patients or those who prefer fully autologous treatment, PRP remains an excellent choice.
  • Both can be combined with microneedling. Bespoke also offers microneedling with PRP for patients who prefer that route.

Bespoke’s Exosome Protocol

Standard treatment-day workflow:

  1. Topical anesthetic applied for 20–30 minutes
  2. Microneedling, microchanneling, or RF treatment per indication
  3. Exosome serum applied directly to treated tissue while channels are open
  4. Post-treatment occlusive recovery sheet for 60 minutes
  5. Take-home recovery skincare provided

Total chair time: 60–75 minutes. Visible improvement begins at 2–4 weeks; collagen-mediated remodeling continues for 12–16 weeks per session.

Who Is a Candidate?

Exosome therapy is appropriate for adults seeking regenerative skin or scalp results who:

  • Are not pregnant or breastfeeding
  • Have no active malignancy or autoimmune flare

PRP is concentrated from your own blood, autologous and variable in quality based on your physiology that day. Exosomes come from young donor MSCs, standardized, consistent, and often more potent for patients over 50 or with depleted regenerative reserves.

Most facial-rejuvenation and hair-restoration protocols use a series of 3 sessions spaced 4–6 weeks apart, then maintenance every 6–12 months depending on goals.

Skin redness/sensitivity for 24–48 hours after microneedling+exosome combinations, similar to a strong sunburn that fades by day 3. Most patients are camera-ready in 3–5 days.

Candidacy for scalp protocols is determined at your consultation with Dr. Indianos. Microchanneling with exosome application is a Bespoke protocol that can be combined with oral or topical regimens when appropriate for compounding effect.

Properly sourced exosomes have a strong safety record, allergic reactions are rare, and because exosomes carry no nuclear material there is no immune-rejection mechanism. The main acute risk is the microneedling step itself, which Dr. Indianos screens carefully for.
